Wildlife Services Form 37 documents depredation incidents and recommends whether a federal permit should be issued. The form is completed by a WS biologist after a site visit and includes:

If WS Form 37 recommends a permit, you'll apply to U.S. Fish & Wildlife Service (USFWS) for a federal eagle depredation permit under the Bald and Golden Eagle Protection Act.
